Medicare is federal health insurance for people 65 and older, as well as some younger individuals with disabilities. Original Medicare includes two parts: Part A and Part B. Part A covers hospital stays, skilled nursing care, hospice, and some home health services, and is free for most people. Part B covers doctor visits, outpatient care, preventive services, and medical equipment, with a standard monthly premium of $185 in 2025 for most individuals.
If you’re receiving Social Security, enrollment is automatic. Your Medicare card arrives about 3 months before your turn 65, coverage starts the first day of your birthday month. If not, apply at: www.ssa.gov/medicare/sign-up, up to 3 months before turning 65. This begins your 7-month Initial Enrollment Period: 3 months before, the month of, and 3 months after your 65th birthday. Missing this window without employer coverage may result in a permanent penalty.
Medicare alone leaves significant gaps, including deductibles, 20% coinsurance, and no cap on out-of-pocket costs. That’s why it’s important to add either a Medicare Supplement (Medigap) or a Medicare Advantage plan. Both help cover what Medicare doesn’t, offering added protection and peace of mind. The right choice depends on your needs, budget, and lifestyle. (You’ll still pay your Medicare Part B premium with either option.)
